About This Position

HopeHealth is seeking an experienced Chaplain

The Chaplain actively participates as part of an interdisciplinary team to coordinate and provide spiritual care to patients, families and staff, which are accepting, affirming and respectful of diverse belief systems and spiritual practices.

This is a full time 40 hour position working the hours of 8am-4:30pm

What will a Chaplain do?

  • Provides spiritual/pastoral assessment through contact with patient/family within five (5) days of admission, per patient need.
  • Provide spiritual/pastoral care in accordance with the ongoing wishes and needs of the patient/family.
  • Develop and implement an individualized spiritual/pastoral plan of care according to goals and wishes of patient/family.
  • Conduct services of prayer, worship and rituals for patients/families, staff and community members as appropriate.
  • Offer patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values.
  • Document spiritual counseling, and other interventions, provided in the patient's clinical record, according to HPC-RI procedures and policies.
  • Work collaboratively as a member of the Interdisciplinary Team(s) to ensure patient/family needs are met, continually evaluating patient response to care.
  • Facilitate linkage to community clergy or other faith-based leaders as requested by patient/families.
  • Active participation in Interdisciplinary Team meetings, staff meetings, and other meetings as requested and expected for the position.
  • Work collaboratively with Bereavement to plan HPC-RI memorial services and other organizational outreach activities.

What are the qualifications for a Chaplain?

  • Master's Degree in religious or theological studies required
  • One to two units of Clinical Pastoral Education or equivalent training, education and experience
  • One to three years' experience for Chaplain, three to five years' experience for Senior Chaplain, in community service
  • Must be comfortable with prayer in an interfaith environment and respectful of all religious beliefs/spiritualties
  • Knowledge and experience with grief and end of life issues preferred
  • Knowledge of caring for patients and families in the home setting or inpatient facilities preferred
