Responsibilities
Provide direct patient care under supervision of registered nurse or physician.
Administer medications and monitor patient responses.
Collaborate with healthcare team to ensure optimal patient outcomes.
Monitor and report changes in patient conditions to healthcare team.
Document patient information accurately and maintain medical records.
Educate patients and families on health management and procedures.
Ensure compliance with healthcare regulations and standards.
Support team in achieving performance targets and goals.
Assist in developing and implementing patient care plans.
Participate in continuous professional development and training.
Maintain a clean and safe working environment.
Utilize pipelines to assess and improve patient care strategies.
Experience/Qualifications
Valid LVN/LPN license in the state of employment.
Excellent communication skills for patient and family education.
Ability to work in a results-oriented environment with high visibility of work activity.
Strong ability to administer medications and monitor patient responses effectively.
Proficiency in documenting patient information and maintaining medical records.
Demonstrated ability to collaborate with healthcare teams for optimal outcomes.
Commitment to continuous professional development and training.
Knowledge of healthcare regulations and standards compliance.
Experience in developing and implementing patient care plans.
