Job Title: Travel Registered Nurse (RN) – Case Management
Facility Overview
Portsmouth Regional Hospital is a full-service medical facility located in Portsmouth, NH. The Case Management department plays a vital role in coordinating patient care, discharge planning, and ensuring continuity of services across the hospital. The team includes 3 Social Workers and 10 RN Case Managers, supported by a manager and director.

- 3 Social Workers
- 10 RN Case Managers
- 1 Manager and 1 Director
- Patient Load: 25–40 cases per RN
- Documentation Systems: Expanse & CarePort
- Dress Code: Business Casual
- Floating: May float to other assignments as needed
- Weekends: No weekend shifts required
Responsibilities
- Perform inpatient case management including discharge planning, guardianship, and surrogacy coordination
- Collaborate with families and interdisciplinary teams to plan long-term care placement
- Utilize Expanse and CarePort for documentation and care coordination
- Support revenue integrity and utilization management
Requirements
- Experience:
- Minimum 2 years of acute hospital RN case management experience
- Critical care experience preferred
- Licensure:
- Active Compact RN License (required at submission)
- Education:
- Certifications:
- BLS (American Heart Association or Red Cross)
