• MA Pharmacy license

Job Details

• Pharmacist position at Nantucket Cottage Hospital, part of Mass General Brigham system.
• Shifts scheduled between 8:30 AM – 7:00 PM, with weekend requirements.

Job Requirements

• MA Pharmacy license required.
• Primary Source Verification of professional license and/or certification within 30 days of start date and updated upon expiration.
• MA Conditional Licenses not accepted.
• Both NURSYS and BON are required for nursing licenses (note: may not apply to pharmacist but included in original).
• Documentation of immunizations including MMR, Influenza Vaccine (required during flu season Sept 15th - April 30th), TB Test within 3 months of start date, Health Statement within 365 days of start date.
• Criminal Background Check within 45 days prior to start date and updated if >6 month break.
• Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
• MGB Resume including past 7 years of work experience required for submission.
• Respirator Medical Clearance Attestation if fit test required.
• 2 recent references, minimum 1 Supervisor or Above, in skillset required for submission.
• MGB Skills Checklist required for submission.
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
• Education Verification required.
• MGB SSN Trace required.
• MGB National Sex Offender Check required.
• MGB Exclusion Checks required.
• MGB Candidate Submission Coversheet required for submission.

Schedule Information

• Shifts scheduled between 8:30 AM – 7:00 PM.
• Weekend requirements.

Additional Details

• Shared housing provided at no cost.
• MGB Color Vision test required within 365 days of start.
• MGB Nantucket Housing Agreement required upon hire if staying in NCH housing.
• MGB FEMA ICS 100 Training must be completed, no exceptions, does not expire.
• Documentation of two measles, two mumps, and one rubella vaccines or two MMR vaccines or proof of immunity via blood test.
• Strongly preferred Hepatitis B vaccination or proof of immunity or declination.
• Tdap vaccine preferred within 10 years and updated upon expiration.
• Positive PPD requires negative chest x-ray documentation.
• MGB TB Symptom Survey if positive PPD, updated annually.
